Abstract
This study measured the perceptions of the Junior and Senior high school teachers in Manuel Roxas High School on using the Body Mass Index (BMI) and Height for Age (HFA)Auto compute, an application for School Form 8 or Learner's Basic Health and Nutrition Report in school year 2018-2019. To measure perceptions, a survey was conducted through the use of the BMI and HFA questionnaire, an instrument devised to evaluate the user-friendliness, clarity of the purpose, speed of computation, practicality and ease in transmitting instructions of the BMI and HFA Auto compute application to co-teachers. After gathering responses of 30 teachers, high scores were recorded across all statements which indicated an alternative in accomplishing SF8.
