Abstract
The study entitled Online Assessment Using Google Forms Via Google Platform for Grade 9 Mathematics Students was conducted to support online learning particularly in administering online assessments wherein face-to-face classes are restricted due to the COVID-19 pandemic. Online assessment becomes possible through the LMS platform and one of these being introduced by the Department of Education as an industry partner to education is the Google Classroom. One of the useful features of the Google Classroom is the
Google Forms that was originally used as a web survey but it can be utilized in quiz form.
Due to this the teachers can monitor the learner's progress, get immediate feedback, and
record their grades. On the other hand, learners can easily access the said form and submit
their output on time. Also, Parents/Guardians need not worry to come to school to pick and return the learner's output. However, this study was designed for learners whose preferences are under online and blended modalities. Because Google Forms is web-based application access to internet and network devices, and Google account is the main requirement to participate in this kind of online assessment.

The study was conducted at San Francisco High School specifically to grade 9 level with a total population of 1,606 learners. Stratified sampling was employed in 41 sections obtaining 140 responses via a web survey. A qualitative approach was implemented using the Lickert scale, 5 is the highest and I is the lowest.

A z-test was used to calculate the value then compared to the z-test tabulated value. Based on the computed value decision rule accepts the null hypothesis that is the study is effective in terms of the following criteria: accessibility, organization, security, time management, and user friendly.

enrichment activity.

Classroom Assessment is an assessment conducted by the teachers to track and measure
learner's progress and to adjust instructions accordingly. (DepEd Order titled Policy
Guidelines on the Implementation of Learning Delivery Modalities for the Formal Education)
In this study, it is derived from IAs (summative test) transcribed to online assessment via
Google Forms.

Embedded means "drawn from and linked to an external source but displayed or accessed
locally" (Merriam-Webster Dictionary). In this study, a link from Google form is copied and shared in the Google Classroom or social media platform for the learner's to access the online
assessment.
